The Integrated Service Delivery Team is a program of Trinitas Hospital
that is sponsored by the Division of Developmental Disabilities (DDD). ISDT
is designed to address the unique challenges of dually-diagnosed
(developmentally disabled/mentally ill) individuals who are living in the
community.
The Integrated Service Delivery Team (ISDT) is a multidisciplinary team.
ISDT will address the mental health and behavioral issues of these
individuals. ISDT will assist the families in dealing with the crises and
stressors that they face. We will work in collaboration with the Division of
Developmental Disabilities (DDD) and other service providers in delivering a
coordinated treatment approach. ISDT will utilize its' experience and
expertise with the mental health system to facilitate access to these
services. ISDT will work together with DDD and the Mental Health system to
foster cooperative interaction in the delivery of services to this
population.
ISDT has an integrated approach of using both systems of developmental
disabilities and mental health. Our mission is to support and maintain the
community placements of this dually-diagnosed population.
Services
We provide a range of hands-on training and technical support to
consumers, families, provider agencies and caregivers. Our services are
mobile and we will engage the individual on-site in their own environment.
We have linkages with mental health programs in our region and can
facilitate access to these services. We can act as a liaison between health
systems.

Our team is multidisciplinary and have expertise in developmental
disabilities and mental health. The team consists of a Program Director
(LCSW), a Psychiatrist, a Ph.D. Clinical Supervisor, Masters Level
Clinicians (LCSW, MA) and a Behavior Specialist (BA).
Eligibility
Individuals with a dual diagnosis who are referred by the Division of
Developmental Disabilities and live in the Upper Central Region and Lower
Central Regions of DDD.
