General Information
Brother Bonaventure Extended Care & Rehab Center is a not-for-profit skilled nursing center affiliated with Trinitas Hospital. We are a 120-bed facility designed to provide continuous care and support for individuals who require long-term care or sub-acute care before returning home. Our facility, in conjunction with Trinitas, is committed to improving the health status of the people and communities we serve. We strive to promote independence and improve functional capacities to enhance the quality of life of our residents.

Brother Bonaventure is licensed by the State of New Jersey. We are approved by Medicare and Medicaid as well as Managed Care. We offer 24-hour licensed nursing care; direct access to full hospital support services including the Wound Healing Center, Dialysis Center, ophthalmology, podiatry, dental and psychiatric services, physical, occupational and speech therapies, Social Work services, therapeutic recreation and pet therapy; Pastoral Care programs and Religious services; laundry services; and professional hairdresser, barber and specialized dietary services.

Nursing Care
Brother Bonaventure Extended Care & Rehab Center provides 24-hour licensed nursing care under the supervision of our Medical Director. Our Medical Director, in cooperation with our staff physicians, provides excellent care in managing the overall health of our residents.

Our registered and licensed practical nurses, along with our certified nursing aides and licensed PT, OT, and Speech Therapists, assist long-term and sub-acute patients in recovering from and managing various acute medical and orthopedic conditions. Special and individualized care is provided for all residents.

Brother Bonaventure Extended Care & Rehab Center utilizes our full-service Wound Care and Dialysis Centers, located at Trinitas' Williamson Street Campus. We also work in cooperation with our on-site Behavioral Health Services and Psychiatric Emergency Department, for those patients that may require immediate psychiatric attention and/or placement.

Our Social Service and Admissions department is dedicated to providing detailed information regarding the admission and discharge process at Brother Bonaventure Extended Care & Rehab Center. We pride ourselves on serving as liaisons/representatives for our residents as well as extended support to the family members when dealing with different situations.

We work very closely with all Social Workers, Discharge Planners, Physicians, Family Members, and all other referral sources to ensure that a resident's transition to our facility is made as easily as possible and in a timely manner.

We are dedicated to providing the extra attention and support that is needed for our residents to reach their optimal level of functioning.

Recreation
The resident is the nucleus of our family at Brother Bonaventure Extended Care & Rehab Center. The Recreation Department focuses on maintaining the recreational well-being of each resident while promoting individual independence. Our goal is to ensure that our residents are mentally and physically stimulated in a safe and nurturing environment. The staff works closely with each resident to create a more homelike and family atmosphere. Activities are designed to challenge and stimulate, and focus on assisting the resident to maintain a more active role in their daily lives. Our staff realizes that each resident is special with their own unique special needs. We work together to enable our residents to reach their full potential.
