Your decision to enroll in WIC is appreciated. You need to bring a
completed WIC Referral for each family member applying for WIC benefits. The
process takes about two hours for one person. Each person applying for WIC
must be present.
What Is WIC?
WIC is a supplemental food and nutrition program funded by USDA. WIC
provides nutritious foods, nutrition education, access to healthcare and
referral to community organizations, breast-feeding education, immunization
information and voter registration.
Who Is Eligible For WIC?

Women-pregnant and up to 6 weeks after delivery. After your baby is
born, you must be certified again for you and the baby.
|

If you are breast-feeding, you are eligible for up to 1 year after
delivery.
|

|
Non-breast-feeding mothers are eligible for up to 6 months after
delivery.
|

|
While WIC recommends breastfeeding for all infants, non-breast-fed
infants receive iron-fortified formula until 12 months of age.
|

|
All children are eligible under 5 years of age.
|
To Enroll In WIC You Must:

Meet the WIC income guidelines. For example, from July 2002 through
June 2003, a family of four whose income is lower than $644.00 per week
qualifies for WIC services.

Have nutritional/health needs. The WIC nutritionist will determine
your health/nutritional need.
|

|
Reside in the service area.
|
On your appointment date you need to bring the following:

WIC Referral Form completed by the healthcare provider for each
person applying for WIC (Example: A breast-feeding mother needs to bring
in a Woman's WIC Referral form for herself and a Child's WIC Referral
form for the baby)

Proof of income (pay stub, unemployment stub, Medicaid stub,
disability stub, etc.);
|

Proof of Address (letter or bill mailed to you; rent receipt or
other);
|

Proof of identity for each person applying for WIC (WIC ID folder,
birth certificate, social security or other);
|

Child's immunization record/card. Each child must be present.
